INDIVIVIDUALS UNDER AGE 18 WITH CEREBRAL PALSY 

(1) Individuals under age 18 with cerebral palsy must have a comprehensive examination by a licensed physical therapist or physiatrist prior to his or her whole bone marrow treatment and submit the results of this exam or exams before said treatment is administered. This examination must be written out clearly and submitted prior to the procedure. The exam must include range of motion, strength testing of all extremities, both arms and legs maximum muscle group circumferences, assessment of muscle tone in all four extremities including wrists, ankles, toes and fingers. If there is a cognitive or speech issue these must be mentioned, recorded and submitted prior to the bone marrow treatment. The report must be signed by a licensed physical therapist or physiatrist who is not be related to the patient in any way nor should the parents be a personal friend of the examiner to prevent bias.

(2) These exams must be repeated after ninety days but no longer than six months following the child’s whole bone marrow treatment and the results promptly mailed by certified mail to Dr. Steenblock’s Clinic. The physiatrist or physical therapist’s notes must comment in their report that they certify under penalty of law that their examination failed to show any noticeable improvement in any of the bodily functions tested previously as compared and contrasted to the examination done prior to the bone marrow treatment (Article 1 above).

(3) Dr. Steenblock’s Clinic will refund the full amount paid by the parents or guardian(s) of the treated minor within sixty days after receipt of all exams and notarized documents specified in articles 1-2 above.

To facilitate evaluations of this technique Dr. Steenblock’s Clinic will remit $1000.00 to the person who originally paid for the treatment if we receive an evaluation and report from the original physical therapist or physiatrist (See article 1 above) prior to the bone marrow treatment and then again receive a similar evaluation and report 3 to 6 months after the bone marrow treatment. The report must contain a statement concerning exactly and precisely what changes negative or positive occurred during the time lapse between the two examinations.

(2) These exams must be repeated by the same orthopedic surgeon after ninety days but not later than six months following the child’s whole bone marrow treatment. The orthopedic surgeon’s signed report attesting to the fact that the child did not have any improvement in their treated joint(s) must be send by certified mail to Dr. Steenblock’s Clinic.

(3) Dr. Steenblock’s Clinic will refund the full amount paid by the parents or guardian(s) for the treated minor’s bone marrow treatment within sixty days after receipt of all exams and signed documents specified in articles 1-2 above.

ADULTS (OVER AGE 18) WITH JOINT ISSUES

(1)  Individuals over age eighteen (18) with medically diagnosed joint issues must have a carefully charted/recorded examination by a licensed orthopedic physician of the injured or damaged joint or joints prior to his or her whole bone marrow treatment and submit the results of this exam or exams along with an X-Ray, CT or MRI of the disease-affected joint or joints before said treatment is administered.

(2)  These exams must be repeated by the same orthopedic surgeon after ninety days but not later than six months following the person’s whole bone marrow treatment. The orthopedic surgeon’s signed report attesting to the fact that this person did not have any improvement in their treated joint(s) must be send by certified mail to Dr. Steenblock’s Clinic.

(3) Dr. Steenblock’s Clinic will refund the full amount paid by the adult patient within sixty days after receipt of all exams and notarized documents specified in articles 1-2 above.

CONCERNING DISPUTES

Any dispute concerning patient compliance with the terms of this guarantee is subject to binding arbitration under the rules and auspices of the American Arbitration Association or other mutually agreeable arbitrator.
